Some facts: at the IMO 2009 there were about 560 contestants, among them about 50 girls (15 girls got medals).
Why Huh?

A previous discussion of this and related issues. It's political and controversial, but I'd start with general social inequality and the messages it sends discouraging girls from pursuing math.

On the gender-gap thread mentioned by jmerry, I have posted a brief comment on the article "Cross-Cultural Analysis of Students with Exceptional Talent in Mathematical Problem Solving," by Titu Andreescu, Joseph A. Gallian, Jonathan M. Kane, and Janet E. Mertz, which appeared in Notices of the American Mathematical Society, Vol. 55, No.10, pages 1248-1260, November 2008. I recommend this article highly, if you are interested in the topic of participation by girls in the IMO.
